Privacy Rights Request Form

Important Information

Use this form to exercise a privacy right or make a complaint about the use of your personal data. You do not have to use this form if another legally valid request method is available. Please provide enough detail for us to understand and locate the relevant information, but do not send unnecessary sensitive data or a full identity-document copy unless we specifically request it through a secure channel.

We may ask for proportionate information to verify identity or authority. This protects personal data from unauthorised disclosure. We will not ask for more information than reasonably necessary for that purpose.

E. Details of Your Request or Complaint

Describe the information, communication channel, approximate dates and outcome you are seeking. If this is a complaint, explain what happened and how you would like Solara to address it.

F. Preferred Response Method

If the response contains sensitive or extensive information, Solara may use a more secure delivery method after discussing it with you.

G. Representative Requests

Complete this section only if you act for another person. Solara may ask for evidence of authority, such as a valid power of attorney or another legally acceptable document.

H. Supporting Information

List any documents you attach. Redact unrelated information where possible. Do not send original documents.

I. Declaration

I confirm that the information in this form is accurate to the best of my knowledge and that I am the person identified above or am lawfully authorised to act for that person. I understand that Solara may request proportionate verification information and may refuse or limit a request where the law permits.

J. How to Submit

For a KVKK application, Solara will respond as soon as possible and no later than 30 days. Where EU or UK data-protection law applies, the relevant statutory timetable applies. A UK data-protection complaint, where that procedure applies, will be acknowledged within 30 days and handled without undue delay. We may pause a rights-request timetable only where and to the extent the applicable law permits, for example while reasonably necessary clarification or identity information is awaited.
